Transition to Home: Help On Your Road to Recovery

After a hospital or rehabilitation discharge, you’re sent home. But sometimes, you aren’t quite ready to live independently again. You need a little extra time and extra help before you feel strong and confident enough to be on your own. Providence can help with our Transition to Home program.

Our staff will assist with any activities of daily living you’d like help with, and we'll provide many other services you need. Meanwhile, you’ll have access to all of the amenities our residents enjoy: three chef-cooked meals a day, life-enriching programming, and our beautiful campus. You can feel at ease, knowing healthcare staff are available when you need them.

The specifics of the program are decided by your needs, but the goal is always to get you ready for going back home.
